Participant Waiver and Release

I voluntarily choose to participate in the identified pickleball event and related warmups, practices, travel within the venue, and activities (the “Event”). I represent that I am physically able to participate, will use appropriate equipment, and will stop and seek help if I experience unsafe conditions or symptoms. I am responsible for obtaining medical advice appropriate to me.

I understand that pickleball and event venues involve known and unknown risks, including falls, collisions, impact from balls or paddles, equipment or surface failure, weather, heat illness, communicable disease exposure, conduct of other participants, serious bodily injury, disability, property loss, and death. I knowingly accept and assume risks inherent in participation and risks that are reasonably foreseeable from the Event, to the fullest extent permitted by law.

To the fullest extent permitted by law, I release the event organizer identified in my registration, participating venue owners and operators, sponsors, sanctioning bodies, officials, volunteers, and Pickleball Parlor, and each of their respective officers, employees, and agents (the “Released Parties”), from claims for personal injury, death, or property damage arising from ordinary negligence connected with the Event.

• This release does not apply to gross negligence, reckless or intentional misconduct, or any liability that applicable law does not allow to be released.
• It does not waive product-liability, consumer, workers’ compensation, or other rights that cannot lawfully be waived.
• It does not make Pickleball Parlor responsible for an independent organizer’s venue, staffing, supervision, or safety decisions.

I will follow event and venue rules and lawful safety instructions. If I cannot consent during an emergency, I authorize reasonable emergency evaluation, first aid, transportation, and treatment. I understand that responders and medical professionals exercise independent judgment and that I am responsible for resulting charges to the extent allowed by law.

I am responsible for my personal property. The Released Parties are not responsible for theft, loss, or damage except to the extent caused by conduct for which liability cannot lawfully be limited.

Promotional use of my image, voice, or likeness is optional and is not a condition of participation. The signing form presents a separate choice. If granted, permission covers Event-related editorial and promotional use without compensation; it may be revoked for future uses by written request, but revocation cannot undo material already published or commitments already made.

If the participant is under the age of majority, a parent or legal guardian must sign. The signer represents that they have legal authority to consent for the minor, agrees to the risk acknowledgment and release on the minor’s behalf to the extent permitted by law, and accepts responsibility for ensuring appropriate supervision and compliance with Event rules.

Typing the signer’s name, selecting signer capacity, affirmatively checking the acceptance box, and submitting the form constitute the signer’s electronic signature. The system records the document version, timestamp, account, IP address, user agent, Event, signer capacity, and signature. Event-specific terms displayed with the form are incorporated. If a provision is unenforceable, it will be limited to the minimum extent necessary and the remainder will continue to apply.
